My tree is dying or dead. Help!!

I planted this tree about a year ago when I bought my house. It was a special moment because I planted it with my family. It made it through the winter and was doing well all year. But last week, I added some fertilizer from the dollar store, the small rocks, and now it looks terrible. I really regret doing that because it was healthy before.
Since then, I’ve removed the fertilizer and watered it, but the leaves are still very brown, and I’m worried it might be too late. Is there anything else I can do to try to save it? Also, I’m not even sure what kind of tree it is—could you help me identify it?
I’d really appreciate any advice. The tree has sentimental value to me and my family, and I’d love to do what I can to bring it back.
