30yr old Struggling shefflera

This plant is almost 30 years old, it’s been in and out of traumatizing situations, but has had some great rebounds. It was doing well up until a few months ago, and it started shedding its leaves like crazy and started to look like the pictures. Any new growth turns brown and dries out and dies within a few days of showing up. I’ve been making sure the soil is staying hydrated though the growing season and the heat and just added some fertilizer a few weeks ago and it still continuing to drop leaves. I Would love some insight from the pros here in this thread.

It is getting watered once a week, and I fill it until it drains through and let it sit in the overflow pan for 15 to 20 minutes to saturate the soil before draining the pan. It has been getting south facing and indirect sunlight for 8 to 12 hours a day.
